The first time you visit our office as a
new patient, we will need to make sure
that we have all of your personal
contact and medical history information.
You will usually receive your new
patient paperwork in the mail before
your visit.
Please fill out your paperwork
before you arrive for your appointment.
If you have not completed your
paperwork prior to your arrival, please
arrive at least 15 minutes early for
your appointment.
Please bring any significant records
with you regarding your medical history
– this applies especially to patients
who have had previous surgeries (i.e.
cleft lip or palate repairs, breast
implants, and breast cancer) and those
with chronic medical conditions. You
will also need to bring a complete list
of the names and doses of any
medications that you are currently
taking.
If we do not have all of the
necessary information, your appointment
may have to be rescheduled.

Once you have been called back for
your appointment, you will first spend
time with our nurse.
During this time, you may be
asked to elaborate on your medical
history information. You will receive
paperwork regarding the surgery of your
interest; you may take this home for
review.
You may also watch a PowerPoint
presentation containing information
related to your surgery.
When you see the doctor, he will discuss
the procedure with you and answer any
questions you may have.
He will also evaluate whether or
not you are a good candidate for
surgery.
Please note that patients with
certain medical conditions may not be
eligible for elective cosmetic
procedures.
